What Is a Scalp Analysis?
A scalp analysis is a magnified examination of the scalp surface and the hair emerging from it. A handheld or digital scope magnifies the scalp many times beyond what the eye can resolve, and what becomes visible at that magnification is the difference between a guess and an assessment.
In dermatology the technique is called trichoscopy, and it is described in the literature as a simple, non-invasive method for evaluating hair loss that can be performed with a manual dermatoscope at around ten times magnification or a video dermatoscope at considerably higher.
The important point is what it replaces. Without magnification, assessment relies on how the hair looks in a mirror and how much appears in the shower drain — both of which are poor guides, and both of which tend to register a problem only once a substantial proportion of density has already gone.

What a Scalp Analysis Measures
Each of these tells you something the mirror does not. Together they distinguish between the two situations that look identical from the outside — hair that is thinning because follicles are shrinking, and hair that is shedding because a temporary phase has pushed follicles into rest at once.
Those two require different responses, and treating one as though it were the other wastes months.
The Single Most Useful Reading: Shaft Diameter Variability
If a scalp analysis produces one finding that changes decisions, it is usually this one.
In a healthy scalp, hair shafts are broadly similar in thickness. In androgenetic alopecia, follicles miniaturise progressively — each growth cycle produces a slightly finer, shorter hair — so the scalp comes to carry a mix of thick, medium and fine shafts side by side.
A systematic review of trichoscopy in androgenetic alopecia found hair diameter variability in around 94 per cent of patients, with vellus hairs present in roughly two thirds and the peripilar sign in over 40 per cent. That makes diameter variability the most consistent single marker of pattern loss available without a biopsy.
It also explains why people are so often surprised by their results. Miniaturisation is visible under magnification long before it is visible in the mirror — which is precisely the window in which treatment has most to work with.
What the Findings Typically Mean
The last of those is worth emphasising. A scalp analysis is a screening and matching tool, not a diagnosis of every condition — part of its value is identifying what needs referring elsewhere.
Why Baseline Photography Matters as Much as the Reading
The measurement taken on day one is only half of what a scalp analysis is for.
Hair changes too slowly to perceive. Nobody can reliably recall how dense their parting was three months ago, and the tendency is to see either no change at all or dramatic change depending on the day and the lighting. Standardised photographs at the same zones, magnification and lighting remove that problem entirely.
This is the practical reason to insist on imaging before starting any treatment anywhere. Without a baseline, the assessment at month three is a conversation about impressions. What a Scalp Analysis Cannot Tell You
A magnified assessment is genuinely useful, and overstating it does nobody any favours. There are clear limits to what looking at the scalp surface can establish.
There is also an interpretive limit worth stating plainly. Trichoscopic findings overlap between conditions — vellus hairs, for instance, appear in both androgenetic alopecia and recovering alopecia areata. The findings inform a judgement; they do not announce a diagnosis on their own, which is why the person interpreting them matters as much as the equipment.
