Low Ferritin Hair Loss in Singapore: Why "Normal" Iron Results Can Still Mean Shedding

What Is Low Ferritin Hair Loss?

You are shedding more than usual, so your doctor orders a blood test. Your haemoglobin comes back normal and you are told you are not anaemic. So iron cannot be the problem, right? Not necessarily.

Low ferritin hair loss is easy to miss because iron deficiency and iron-deficiency anaemia are not the same thing. Haemoglobin measures the oxygen-carrying protein in your red blood cells. Ferritin is a protein that stores iron, so a ferritin blood test is used as a marker of your iron reserves.

Those reserves can run down before haemoglobin falls far enough for you to be called anaemic. Think of ferritin as a reserve tank: it empties first, and haemoglobin only drops later if the shortfall continues.

That does not mean everyone who sheds needs iron, or that every borderline result explains hair loss. It means persistent diffuse shedding deserves a cause-first assessment. For a wider view of what helps, see our guide to hair loss treatment options.

Why iron stores matter to hair

Growing hair follicles are some of the fastest-dividing cells in the body, and iron is involved in many of the processes they rely on. You can read more about how the growth cycle works in About Hair Loss & Hair Growth.

Iron deficiency is a recognised trigger of telogen effluvium, a diffuse, non-scarring shedding where more hairs than usual enter the resting phase at once. It rarely causes one sharply defined bald patch. Instead you notice more hair in the shower, on the pillow or in the brush, a smaller ponytail, and more scalp showing overall.

The timing is confusing. Shedding usually becomes obvious two to four months after the trigger, so a heavy period, restrictive diet or illness months ago may not seem connected.

What Ferritin Level Is Too Low?

There is no single number that fits every situation. Ferritin is reported in µg/L or ng/mL; for ferritin the two are numerically the same, so 20 ng/mL equals 20 µg/L.

Some guidelines use higher cut-offs in specific settings; gastroenterology guidance, for example, uses 45 ng/mL when diagnosing iron deficiency in people who are already anaemic. That is a diagnostic threshold, not a hair-growth target, and it is often quoted online as if it were.

What the research actually shows

A 2022 systematic review of 10,029 women found that women with non-scarring hair loss had ferritin about 18.5 ng/mL lower on average than women without it. Where very low ferritin was used to define deficiency, about 21% of women with hair loss met the threshold. The studies varied a lot, though, and none established a universal target.

So claims like "your ferritin must be above 70 for hair to grow" should be treated with caution. A clearly deficient ferritin needs medical attention, but ever-higher ferritin does not mean ever-more hair.

Why a "normal" ferritin can mislead

Ferritin rises with inflammation. Someone can be short of usable iron while their ferritin looks normal because another process is pushing the number up. In that case a doctor may look at transferrin saturation (TSAT), where a result below about 20% can support iron deficiency, alongside the full blood count and an inflammation marker such as C-reactive protein.

The same goes for "my iron test was normal". Serum iron fluctuates through the day and is not read on its own. Normal haemoglobin, normal serum iron and normal ferritin are three different statements.

Why Ferritin Drops, and Which Tests to Ask For

Finding low iron stores is only half the job. The next question is why.

[Periods] Heavy or prolonged menstrual bleeding

Repeated blood loss means repeated iron loss. If periods have become heavier or longer, replacing iron without addressing the bleeding often means the deficiency returns.

[Pregnancy] Pregnancy and the postpartum period

Iron needs rise in pregnancy and delivery adds blood loss. Postpartum shedding is also common on its own, so several triggers can overlap.

[Diet] Restrictive eating or rapid weight loss

Calorie restriction can reduce iron intake, and rapid weight loss is itself a telogen effluvium trigger. A well-planned vegetarian diet can supply enough iron, but absorption needs attention.

[Blood loss] Blood donation, gut problems or malabsorption

Frequent donation, coeliac disease and other gut conditions can deplete stores. In men and post-menopausal women, unexplained iron deficiency usually needs investigating for a source rather than taking iron indefinitely.

Why women are asked about ferritin so often

Monthly blood loss raises iron needs for much of adult life, pregnancy adds more demand, and dieting can reduce intake. In the 2022 review above, the ferritin gap between women with and without hair loss was even larger in premenopausal women.

But low ferritin should not become a catch-all explanation for female hair loss. The same woman can have female pattern hair loss, telogen effluvium, thyroid changes, PCOS-related androgen effects, stress and scalp inflammation at once. That is exactly why a cause-first approach beats buying another "hair growth" product.

Which blood tests to ask about

If diffuse shedding persists, speak to a GP or dermatologist. Depending on your history, the core tests are:

Other tests should follow your history, because diffuse shedding can also come from thyroid problems, illness, weight loss, medication or other deficiencies. A longer checklist is not automatically better.

What to Expect: Treatment and Recovery Timeline

If iron deficiency is confirmed, the priority is to correct it and find out why it happened. Doctors usually start with oral iron and consider intravenous iron when tablets are not tolerated, do not work or are unlikely to be absorbed.

Do not start high-dose iron just because you are shedding. Too much iron can be harmful, supplements can upset the stomach, and self-treating can hide the reason ferritin fell in the first place.

Treat the reason as well as the number

If heavy periods keep draining iron, the bleeding needs assessing. If a restrictive diet is responsible, the eating pattern needs correcting. If absorption is the problem, simply raising the tablet dose may not help, and if there is hidden blood loss, the source needs finding. Fixing ferritin while ignoring the cause often brings a temporary improvement followed by another drop.

Hair recovery lags behind blood recovery. Hairs already in the resting phase keep falling for a while after the trigger is fixed.

If ferritin improves but your hair does not

Correcting a real deficiency matters for your health, but it does not prove iron was the only cause. A bout of shedding can unmask pattern hair loss, and other factors can coexist, including thyroid problems, PCOS, scalp inflammation, medication and chronic telogen effluvium.

If thinning continues once your iron stores are back in range, get it reassessed rather than taking more and more iron.

How Anagen Scalp Supports Recovery

Anagen Scalp does not diagnose iron deficiency, prescribe iron or replace your GP or dermatologist. If low ferritin is suspected, start there.

Once the deficiency is being managed, a magnified scalp assessment can record density, differences in hair thickness, visible regrowth, oiliness, build-up and irritation. That baseline matters because daily mirror checks are a poor way to judge slow recovery. Repeating the same views every few months shows whether new hairs are coming through long before it is obvious in photos.

Where non-invasive scalp care suits you, the Signature Plasma Scalp Boost uses ionised plasma energy to carry exosome-based actives into the scalp dermis, followed by low-level laser therapy (LLLT). It is drug-free with no downtime, and works alongside medical treatment, never instead of it. Learn more about the Plasma Scalp Boost.

The order matters: find the cause, treat the deficiency, then support and monitor the scalp.

Frequently Asked Questions

Can low ferritin cause hair shedding without anaemia?

Yes. Iron stores can run down before haemoglobin falls below the threshold for anaemia. This is called iron deficiency without anaemia.

Is a ferritin of 20 low?

It is above the WHO's below-15 threshold for healthy adults but below the under-30 threshold many clinicians use for depleted stores. Your doctor should read it alongside your symptoms, haemoglobin, inflammation and other iron markers.

Does ferritin need to be above 70 for hair growth?

There is no universally accepted evidence that everyone needs ferritin of 70 for hair to grow. Research links lower ferritin with some non-scarring hair loss, but studies use different thresholds and none sets one hair target.

Should I take iron if my hair is falling out?

Not until a blood test confirms you are iron deficient. Hair loss has many causes, and taking iron you do not need will not fix the real trigger and can be harmful.

Will my hair grow back after low ferritin is treated?

If iron deficiency was a key trigger and the follicles are not scarred, shedding usually improves once the deficiency and its cause are corrected. Recovery is gradual, and blood results improve before hair looks visibly denser.

Low ferritin hair loss is worth checking because iron stores can be depleted even when haemoglobin is normal. Just as important: not every case of shedding is iron, and there is no proven rule that everyone needs ferritin of 70 or 100 to regrow hair.
